Colorado Mental Health law requires clinical social workers to keep certain information on file. Please print out the demographic information and mandatory disclosure forms and bring them to our first session.  If you would like me to coordinate your care with another therapist, medical professional, nutritionist, or school, please fill out the attached release form as well.  I am happy to respond to any questions or concerns you may have about these forms now or at any point in the future.
